• FIR/NCR of the lost license
• Challan clearance report from Traffic Police (in case of Commercial license renewal).
• A Fee of Rs.200.Form No 1. Self Declaration of Physical Fitness.

• Present address,
• Father’s name,
• License number,
• License authority,
• Validity period,
• Class of vehicle,
• Number and date of authorization,
• Whether the authorized to drive transport vehicles if so, the date and authority,
• Serial number of badge if any,
• Endorsement of punishment,
